В психиатрии роль разговора более важна, чем в соматической медицине, поскольку именно в разговоре пациента может выразиться болезнь. Некоторые элементы интеракционной организации изучались нами на материале диагностических бесед в психиатрической больнице. Выявлен повторяющийся паттерн, включающий в себя вопросы врача и ответы пациента. Э. Щеглов (2007) показал, что вопросы с «известным ответом» образуют характерную последовательность из трех частей: вопрос-ответ-оценка. В диагностических интервью в психиатрии вопросы касаются не знаний пациента, а его способности выполнить задачу, при этом не на все тестовые вопросы есть готовый правильный ответ, а результат оценки не всегда объявляется врачом. В рамках выявленного паттерна врач повторяет ответ пациента, не оценивая его, а затем добавляет уточняющий вопрос. Часть повторяющихся циклов в последовательности взаимодействия врача и пациента демонстрирует следующую закономерность: вопрос врача &gt; ответ пациента &gt; врач повторяет ответ пациента &gt; (опциональный уточняющий вопрос врача). Когда же вопросы врача направлены на получение реальной информации, а не просто на проверку способностей пациента, он чаще прибегает к переформулировке ответов пациента вместо того, чтобы просто повторять их; эта переформулировка может работать как уточняющий вопрос. В рамках этого паттерна вопросы в клиническом интервью включаются в две линии «отчетливости»: во-первых, они следуют обычному разговорному порядку, а во-вторых, порядку, обеспечиваемому профессиональными рассуждениями, направленными на проверку диагностических гипотез. Этот второй порядок неочевиден для пациента.</p></abstract><trans-abstract xml:lang="en"><p>Medical doctors arrive at a diagnosis after considering various types of evidence, including the patient's complaints, their outer appearance, talk, and non-verbal behavior. In psychiatry, the role of conversation is more importantthan in somatic medicine, because itisin the patient'stalk thatthe disease can expressitself. Some elements of the conversational machinery have been studied in diagnostic interviews in a psychiatric ward. In the interviews, a recurrent pattern of the doctor’s questions and the patient’s answers was found. Schegloff (2007) showed that “known answer” questionsform a distinctive three-partsequence type: question-response-evaluation. In diagnostic interviewsin psychiatry, the questions don't address knowledge but the capacity to perform the task. Not all the test questions have a correct answer, and the result of the evaluation is not always announced by the doctor. The doctor consistently repeatsthe patient’s answer without evaluating it and then adds a follow-up question. At least parts of the repeated cycles in the sequence of doctor-patient interaction exhibit the following pattern: Doctor’s question &gt; Patient’s answer &gt; Doctor’s echo &gt; (optional Doctor's follow-up question). When the doctor's questions seek real information — and do not just test capacity —they sometimes resort to a reformulation of the patient’s answersinstead of just repeating it; this reformulation works as a prompt. The questions in the clinical interview appear to follow two lines of accountability: the ordinary conversational order and that which is provided by the professional reasoning aimed at checking diagnostic hypotheses.
